NonResident Membership

The LifeMed Alaska Membership is also available to non-residents who work or vacation in Alaska. As long as your medically-necessary transport is performed by LifeMed Alaska and meets the above guidelines, you are covered!

MEMBERSHIP FAQ

I do not have insurance. What are my payment options?

If you do not have insurance, please contact our billing office at 907.249.8360 to discuss the possibility of an uninsured patient discount. Once your eligibility for a discount has been established, you have several options for paying your invoice.

First, mail your payment in full following the instructions on the invoice. Please include the tear-off portion of the invoice so that your account may be properly credited. Secondly, you can call our billing office to pay your balance in full by credit card. LifeMed accepts VISA, Mastercard, Discover Card and American Express.

If you are unable to pay your full balance, please call our billing department to speak with one our friendly representatives to establish a repayment schedule over the phone. Our office hours are from 8am to 5pm, Monday through Friday.

I cannot afford this bill. Will LifeMed consider forgiving or reducing this bill?

We understand that some patients may have difficulty paying for their ambulance service. LifeMed has an established to assist uninsured patients. If you are in need of assistance, please complete and submit the Financial Assistance Request below. LifeMed also honors the Charity determinations made by many Alaskan hospitals. If you have applied for and received a reduction on your bill from the hospital, please contact our billing office at 907.249.8360 to discuss our requirements.

I want to provide my insurance information. How do I do that?

Please complete the Authorization to Bill form below to provide us with your insurance information. We will submit a claim on your behalf and notify you of your coverage level. You may also call our billing office if you prefer to submit your insurance information over the phone, or fill out the back of the invoice with your information and return it to us by mail.

I have insurance. Doesn’t it cover this?

Some insurance companies cover ambulance services, while others will only cover a portion of the charges. We cannot determine the exact coverage you have with your insurance company until we submit a claim to them. Please complete the form to provide us with your insurance information. We will submit a claim on your behalf and notify you of your coverage level. You may also call our billing office if you prefer to submit your insurance information over the phone, or fill out the back of the invoice with your information and return it to us by mail.

Why do you need my signature to bill my insurance?

LifeMed policy requires that we have your signature as authorization to bill and accept payments on your behalf. This signature also allows us to bill any secondary insurance you may have after your primary carrier pays the claim.

I have received multiple invoices for the same date of service. Why?

LifeMed provides both air and ground transportation. Each segment of transportation must be invoiced separately. If you were transported by fixed-wing aircraft (it was not a helicopter) you will most likely receive more than one invoice.
